Liability Release

By checking the box and signing/typing my name in the space provided at the bottom of this document, I agree that I have read this agreement and fully understand its content and meaning, voluntarily agree to all of the terms and conditions below, sign it of my own free will, and I am over the age of 18 years:

I acknowledge that the information provided during life coaching sessions offered to me by Sarah Aguinaldo and Lifeward Choices Empowerment Centre* is limited to that of information and support of a general nature to help me in my quest for improved life experiences.

I acknowledge that Sarah Aguinaldo and Lifeward Choices Empowerment Centre do not dispense medical advice nor prescribe the use of any technique as a form of treatment for mental health issues.

I acknowledge that Sarah Aguinaldo and Lifeward Choices Empowerment Centre do not recommend that any technique be used as a form of treatment to replace professional medical/nutritional/mental health advice or care.

Sarah Aguinaldo and Lifeward Choices Empowerment Centre assume no responsibility for my actions. I acknowledge that I am responsible for my decisions and the outcome of these decisions, while using the life coaching services at Lifeward Choice Empowerment Centre and/or being a registered member of Lifeward Choices Empowerment Centre.

I acknowledge that there are risks associated with participation in the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre. I have informed myself and understand the risks associated with my participation in these sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre on any premises including the risk of personal injury, and I freely accept these risks.

I understand that I am free to withdraw from or reduce our participation in the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre at any time.

I am not aware of any medical condition that would affect my ability to participate in the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre. If I have any concerns about my medical condition, then I will consult with my physician before participating in the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre.

In consideration of the acceptance of my registration for the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre, I hereby for myself, my heirs, executors, administrators, or any others who may claim on my behalf, promise not to sue, and hereby waive, release and discharge Sarah Aguinaldo and Lifeward Choices Empowerment Centre and anyone acting for or on its behalf, from any and all claims of liability for personal injury, illness, loss of life or property damage of any kind or nature, arising out of or sustained in the course of my participation in the sessions, activities and programs offered or sponsored by Sarah Aguinaldo and Lifeward Choices Empowerment Centre on any premises.

This Release and Waiver applies to all claims, foreseen or unforeseen, including negligence and breach of statutory or other duty of care. I agree that Sarah Aguinaldo and Lifeward Choices Empowerment Centre are not responsible in the event of loss, damage, unauthorized use, theft, or injury resulting from and to any personal property that I bring into the Lifeward Choices Empowerment Centre session, activity or program on any premises.

I agree that this Agreement and Release and Waiver is intended to be as broad and inclusive as permitted by law. Any provision found to be invalid or unenforceable by a court shall not affect the validity or enforceability of any other provision.

I have read the above information carefully and acknowledge that I have complete knowledge and understanding of its contents. I recognize that by signing/typing my name and checking off the box below that I am waiving certain legal rights, including the right to sue. I am signing voluntarily.

*Lifeward Choices Empowerment Centre means its founder, employees and life coaches.
